About The Position
Pinnacle Home Care is hiring a PRN Licensed Practical Nurse to provide compassionate, patient-centered home health care. The nurse administers skilled nursing care under RN supervision and performs curative, rehabilitative, or preventive care tasks according to patient care plans. The role involves documenting visits and coordinating schedules while supporting patient rights, quality improvement, and agency policies.
Requirements
- Current state-specific LPN or LVN license.
- Graduation from an accredited nursing program.
- Valid BLS certification.
- At least one year of nursing experience; home health or medical/surgical experience is preferred.
- Excellent communication, organizational, and time-management skills.
- Valid driver’s license and access to a reliable, insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Ability to walk, stand, stoop, and occasionally lift or turn patients.

Responsibilities
- Administer skilled nursing care under RN supervision.
- Perform curative, rehabilitative, or preventive care tasks outlined in patient care plans.
- Use clinical expertise and nursing education to make informed, independent decisions that support patient care and positive outcomes.
- Complete visit reports promptly and accurately in the electronic medical record, including required documentation for care quality and regulatory compliance.
- Confirm daily visit schedules to coordinate smoothly with other staff.
- Understand the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
- Follow the agency’s process for managing and addressing customer complaints.
- Uphold patient rights in accordance with agency service agreements and admission policies.
- Understand agency policies for appropriate patient discharge and reasons for discharge.
