About The Position

Pinnacle Home Care is hiring a Licensed Practical Nurse (LPN) to deliver compassionate, patient-centered home health care. The role provides skilled nursing care in patients’ homes under RN supervision, following patient care plans and supporting positive outcomes. The nurse documents visits in the electronic medical record, coordinates schedules with other staff, and follows agency policies and quality, complaint-management, patient-rights, and discharge procedures.

Requirements

• Current state-specific LPN or LVN license.
• Graduate of an accredited nursing program.
• Valid BLS certification.
• At least one year of nursing experience; home health or medical/surgical experience is preferred.
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Nice To Haves

• Home health or medical/surgical nursing experience.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
• Use clinical expertise and nursing education to make informed, independent decisions that support patient care and positive outcomes.
• Complete visit reports promptly and accurately in the electronic medical record, including all required documentation for quality care and regulatory compliance.
• Confirm daily visit schedules to coordinate care with other staff.
• Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Follow the agency’s process for managing and addressing customer complaints.
• Uphold patient rights in accordance with agency service agreements and admission policies.
• Follow agency policies regarding appropriate patient discharge and reasons for discharge.
