About The Position

Pinnacle Home Care is hiring a PRN Licensed Practical Nurse to deliver compassionate, patient-centered home health care. The LPN administers skilled nursing care under RN supervision and provides services in patients’ homes in accordance with their care plans, agency policies, and applicable documentation and compliance requirements. The role involves coordinating visits and supporting positive patient outcomes while respecting patient rights and the agency’s quality and complaint-management processes.

Requirements

• Current state-specific LPN or LVN license.
• Graduation from an accredited nursing program.
• Valid BLS certification.
• At least one year of nursing experience; home health or medical/surgical experience is preferred.
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Nice To Haves

• Home health or medical/surgical nursing experience.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
• Use clinical expertise and nursing education to make informed, independent decisions that support patient care and positive outcomes.
• Complete visit reports accurately and promptly in the electronic medical record, including required documentation.
• Confirm daily visit schedules to coordinate with other staff.
• Maintain awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Follow the agency’s process for managing and addressing customer complaints.
• Uphold patient rights in accordance with service agreements and admission policies.
• Follow agency policies concerning appropriate patient discharge and reasons for discharge.
