About The Position
This is a travel Licensed Practical Nurse position in Long Term Care, specifically within a Skilled Nursing Facility. The contract is for 8 weeks, with a start date of September 1, 2026. The role involves providing direct nursing care, administering medications and treatments, and collaborating with an interdisciplinary team. The position requires availability for evening shifts (5x8) with guaranteed 40 hours per week.
Requirements
- Active Licensed Practical Nurse license in New Hampshire, in good standing.
- Ability to perform nursing duties in a long-term care setting and follow physician orders.
- Availability for evening shifts with a 40-hour per week schedule.
- Strong communication and documentation skills; capable of working with an interdisciplinary team.
- Must be able to meet background check and health screening requirements as required by practice regulations.
Responsibilities
- Administer prescribed medications and treatments per physician orders.
- Provide direct nursing care to residents, including monitoring for changes in condition.
- Document assessments, vitals, care plans, and interventions accurately in the charting system.
- Collaborate with the interdisciplinary team to coordinate resident care and ensure safety and quality standards.
- Assist with daily living activities, wound care, and routine nursing procedures as appropriate.
- Uphold regulatory compliance, safety, and infection control practices in a long-term care environment.
