About The Position

This is a 13-week travel assignment for a Licensed Practical Nurse (LPN) in Long Term Care, located in Bennington, Vermont. The role is within a skilled nursing facility and requires the nurse to work evening shifts. The assignment is through Healthforce Healthcare LLC.

Requirements

• Active Licensed Practical Nurse license in Vermont (in good standing)
• Graduation from an approved LPN program (diploma or certificate)
• Relevant experience in long-term care or skilled nursing settings preferred
• Ability to work the stated evenings shift (15:00–23:30) and weekends; 32 guaranteed hours per week
• Strong clinical skills, communication abilities, and ability to work in a skilled nursing environment
• Standard credentialing and health compliance requirements typical for travel assignments

Responsibilities

• Administer prescribed medications and provide routine nursing care under supervising staff
• Monitor patient status, document vitals and changes, and report findings to the supervising RN
• Assist with activities of daily living, bathing, dressing, nutrition, and mobility
• Implement and update care plans, including wound care and dressing changes as needed
• Respond to call lights promptly; collaborate with interdisciplinary team to ensure safety and comfort
• Maintain accurate, timely documentation and comply with relevant policies and regulations
