About The Position
Amedisys is hiring a part-time, day-shift Licensed Practical Nurse for home health care. The nurse supports patients healing at home by carrying out the plan of care in coordination with registered nurses and physicians, educating patients and caregivers, monitoring patient status, managing medications, performing ordered procedures, and documenting care.
Requirements
- One year of clinical LPN experience.
- Current LPN license specific to the state or states where assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review all medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by the physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
