About The Position

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. You will manage care plans with a dedicated team of physicians and clinicians to support recovery and help patients regain independence. The coverage area includes the south end of Murrells Inlet (partly in Horry County), Pawleys Island, Georgetown, and Andrews. The role has an associated $5,000 sign-on bonus.

Requirements

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting.

Nice To Haves

• Nursing experience in a Home Health or Hospice setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and their families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient visits, interventions, and outcomes.
• Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
• Participate in care coordination activities and discharge planning.
