About The Position

As a CenterWell Home Health nurse, the Home Health Licensed Practical Nurse provides personalized, one-on-one care to help patients regain independence in their homes. Reporting to the Clinical Manager and working with physicians and clinicians, the nurse manages care plans that support patient recovery. The role provides skilled nursing care in patients’ homes under the direction of an RN and collaborates with patients, families, and the healthcare team to deliver individualized care. The position is patient-facing and requires TB screening. It also participates in Humana’s driver safety program and requires a valid state driver’s license and personal vehicle liability insurance meeting applicable limits. Scheduled weekly hours are 40. The posting lists a pay-per-visit/unit rate of $33.00–$47.00 and a full-time annual base-pay range of $53,000–$73,000. Compensation may vary based on location and job-related skills, knowledge, experience, education, and certifications.

Requirements

• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and families about disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient visits, interventions, and outcomes.
• Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
• Participate in care coordination activities and discharge planning.
