About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. You work with physicians and clinicians to manage care plans that support recovery and help patients regain independence. The role includes implementing individualized care plans with patients, families, and the healthcare team; educating patients and families; documenting care and outcomes; and communicating promptly about patient needs and status changes. You also participate in care coordination and discharge planning. The position involves patient-facing work in patients' homes and is subject to Humana's TB screening and driver safety programs.

Requirements

Current nursing license in the practicing state.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Two years of experience as an LPN/LVN in a clinical setting.

Nice To Haves

Responsibilities
