About The Position

HSHS Rehabilitation Hospital is seeking a Licensed Practical Nurse to provide direct and indirect patient care in a rehabilitation setting. The nurse uses the nursing process of assessment, planning, intervention, and evaluation; develops care plans with patients, families, and interdisciplinary staff; communicates changes in patients’ clinical conditions; and participates in discharge planning. This is a full-time day schedule consisting of three 12-hour shifts. A sign-on bonus of up to $7,500 is offered.

Requirements

• Active Wisconsin or compact-state LPN license; if relocating, licensure must be updated to the new Primary State of Residence before employment begins.
• AHA Basic Life Support (BLS) certification within 30 days of hire.
• Associate's degree required.
• Critical-thinking skills and decisive judgment.
• Ability to work with minimal supervision and take appropriate action in a stressful environment.

Nice To Haves

• One year of previous experience preferred.

Responsibilities

• Plan and deliver direct and indirect patient care using assessment, planning, intervention, and evaluation.
• Develop personalized nursing care plans in coordination with patients, families, and interdisciplinary staff.
• Monitor patients and communicate changes in their clinical condition to physicians, nursing supervisors or managers, and coworkers as appropriate.
• Participate in the discharge planning process to support safe transitions.
