About The Position

The Licensed Practical Nurse (LPN) provides direct and indirect patient care to support patients’ recovery and rehabilitation. The role works with patients, families, and an interdisciplinary care team to develop personalized care plans, monitors patient conditions and communicates changes to relevant clinicians, and supports safe transitions through discharge planning. The position is full-time, working nights on a 3x12 schedule, at Rehabilitation Hospital of Wisconsin. The hospital provides specialized rehabilitation care for people recovering from stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.

Requirements

• Active Wisconsin or compact-state LPN license; if relocating, licensure must be updated to the new Primary State of Residence before employment begins.
• Associate's degree required.
• AHA Basic Life Support (BLS) certification within 30 days of hire.
• Critical thinking skills and decisive judgment.
• Ability to work with minimal supervision and take appropriate action in a stressful environment.

Nice To Haves

• One year of previous experience preferred.
• IV certification highly preferred; otherwise, must obtain it within six months of starting.

Responsibilities

• Deliver direct and indirect patient care using the nursing process: assessment, planning, intervention, and evaluation.
• Collaborate with patients, families, and the interdisciplinary care team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues.
• Participate in discharge planning to support safe, successful transitions.
