About The Position

TGH Rehabilitation Hospital is seeking a Licensed Practical Nurse to support patients during inpatient rehabilitation. The nurse provides direct and indirect care using the nursing process, works with patients, families, and the interdisciplinary care team to develop personalized care plans, monitors patient conditions and communicates changes, and contributes to discharge planning to support safe transitions. The role helps patients regain independence, strength, and quality of life.

TGH Rehabilitation Hospital is an 80-bed inpatient acute rehabilitation hospital providing individualized care for people recovering from injury or illness, including stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury. Requirements

• Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
• Current state licensure as a Licensed Practical Nurse.
• CPR/BCLS certification.
• Excellent oral and written communication and interpersonal skills.

Nice To Haves

• ACLS certification preferred.
• At least six months of Medical/Surgical experience in an acute care setting preferred.

Responsibilities

• Deliver direct and indirect patient care using the nursing process: assessment, planning, intervention, and evaluation.
• Collaborate with patients, families, and the interdisciplinary care team to create personalized care plans.
• Monitor changes in patient conditions and communicate them to physicians, supervisors, and colleagues.
• Participate in discharge planning to support safe and successful transitions.
