About The Position

The Licensed Practical Nurse (LPN) provides care and services to a group of patients, visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. The LPN contributes to the interdisciplinary team by providing expertise in psychosocial assessments and interventions, as well as knowledge of community resources. This PRN position helps cover DeKalb and Gwinnett counties.

Requirements

• Current Licensed Practical Nurse (LPN) license in good standing in the state(s) of practice.
• One year of experience under the LPN license.
• Ability to work well in a team environment and collaborate daily with clinicians.
• Ability to work one-on-one with patients and independently on a day-to-day basis.
• Ability to handle phone, email, and written communication and meetings with a variety of healthcare professionals.
• Understanding of state and federal rules and regulations.
• Time-management skills for visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive; reliable transportation; valid driver’s license; and auto insurance.

Responsibilities

• Provide care and services to a group of patients.
• Visit patients in private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team by providing expertise in psychosocial assessments and interventions and knowledge of community resources.
• Manage patient visits, documentation, and meetings; attend scheduled visits and required meetings; and respond to urgent needs by prioritizing and triaging multiple concerns.
• Collaborate daily with a team of clinicians and communicate with healthcare professionals.
