About The Position

Brighton/Suncrest Hospice is seeking a Licensed Practical Nurse (LPN) to provide care and services to a group of patients. The nurse visits patients where they reside, including private homes, skilled nursing facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, the LPN provides expertise regarding psychosocial assessments, interventions, and knowledge of community resources. This PRN role helps cover Fulton and Cobb Counties.

Requirements

• Current Licensed Practical Nurse (LPN) licensure in good standing in the state(s) of practice.
• One year of experience under an LPN license.
• Ability to work well in a team environment and collaborate with clinicians.
• Ability to work one-on-one with patients and independently on a day-to-day basis.
• Ability to communicate by phone, email, and writing and meet with a variety of healthcare professionals.
• Understanding of state and federal rules and regulations.
• Time-management skills to coordinate visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive, reliable transportation, a valid driver’s license, and auto insurance.

Responsibilities

• Provide care and services to a group of patients.
• Visit patients in their residences, including private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team by providing expertise on psychosocial assessments, interventions, and community resources.
• Manage visits and documentation, attend required meetings, and respond to urgent needs by prioritizing and triaging concerns.
• Collaborate daily with a team of clinicians and communicate with healthcare professionals.
