About The Position

The Licensed Practical Nurse (LPN) contributes to the nursing process of assessment, planning, implementation, and evaluation. Under the supervision of registered nurses, the LPN provides patient care and guidance in patient education and activities consistent with their education and demonstrated competencies. The role serves patients in Health Care Units and housing units and supports individualized treatment plans, medication administration, diagnostic testing, emergency response, and clinical documentation.

The position is part of a mission-driven organization focused on providing compassionate care to underserved and vulnerable patients. Team members are offered training and development opportunities and are expected to attend mandatory staff meetings and training sessions and follow applicable policies and procedures. Applicants must reside in the United States.

Requirements

• Graduate from an accredited school of nursing.
• Maintain current licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in the state of employment.
• Be able to obtain and maintain CPR certification.
• Reside in the United States.

Nice To Haves

• A minimum of one year of clinic experience is preferred.

Responsibilities

• Under RN supervision, assess patients’ physical, psychological, and social dimensions in Health Care Units and housing units, contribute to individualized treatment planning, and consult with RNs and staff as needed.
• Implement treatment programs directed by Health Care Practitioners, administer medications (including IV therapy if certified), and perform diagnostic tests according to established protocols and procedures.
• Participate in controlled substance counts at the beginning of each shift.
• Follow safety techniques, procedures, and standard precautions when responding to codes or health emergencies within standard guidelines.
• Assist with medical or minor surgical procedures and implement nutrition and therapeutic diet plans.
• Document nursing encounters using SOAP charting and communicate effectively with nursing staff, physicians, and other relevant personnel.
• Attend mandatory staff meetings and training sessions and follow policies and procedures.
