About The Position

This is a travel assignment for a Licensed Practical Nurse (LPN) in Long Term Care in Newport, Vermont. The role involves providing direct patient care to residents in a Skilled Nursing Facility, administering medications, monitoring vital signs, assisting with daily living activities, and collaborating with the healthcare team. The assignment is for 14 weeks, starting on 09/01/2026, with a schedule of 3x12-hour night shifts per week, including every other weekend. This is a local-only assignment.

Requirements

Active LPN license (in good standing)
Local candidates only for the Newport, Vermont area
Ability to work night shifts (3x12 hours) and meet the stated schedule
Comfortable working in a long-term care/skilled nursing environment with geriatric residents
Ability to pass background check and credentialing requirements

Responsibilities

Provide direct patient care to residents in a long-term care setting, including routine assessments and assistance with activities of daily living
Administer prescribed medications and treatments per physician orders; document administration accurately
Monitor vital signs, assess changes in resident condition, and report findings promptly to the supervising nurse
Maintain accurate charting and documentation in the electronic health record; ensure continuity of care
Assist with wound care, basic IV care where applicable, and skin integrity; support mobility and fall-prevention efforts
Collaborate with the interdisciplinary team to develop and implement care plans; communicate changes and concerns to the charge nurse or supervisor
Uphold infection control policies, patient safety standards, and residents’ rights
