About The Position

Requirements

Active LPN license (New Hampshire) or eligibility for licensure in the state.
Current CPR/BLS certification.
Ability to work evenings on a 5x8 schedule and to accommodate local-week restrictions as described.
Completed LPN diploma from an approved program; maintain ongoing professional development as required.

Nice To Haves

Prior experience in long-term care or skilled nursing settings preferred.

Responsibilities

Administer prescribed medications and treatments under supervision, documenting delivery according to policy.
Perform routine assessments and ongoing monitoring of residents, identifying changes and communicating with the care team.
Provide direct patient care, including assistance with activities of daily living and mobility, while supporting residents’ comfort and safety.
Participate in care planning and interdisciplinary rounds, updating care plans as needed and ensuring continuity of care.
Document accurately in the electronic medical record, maintaining HIPAA-compliant patient confidentiality.
Respond to call lights promptly, assist with wound care and dressing changes as prescribed, and monitor vital signs and input/output as needed.
