About The Position

This is a travel assignment for a Licensed Practical Nurse (LPN) in Long Term Care, located in Geneva, New York. The contract duration is 24 weeks, with a start date of 09/15/2026. The role is situated in a Skilled Nursing Facility and involves rotating shifts. The weekly pay range is estimated between $1,303 to $1,373.

Requirements

• Active New York LPN license
• BLS certification (AHA or RQI required)
• Minimum 1 year of experience as an LPN, with prior LTC exposure preferred
• Ability to provide care to adult and geriatric residents
• Willingness and ability to work rotating shifts
• Capable of meeting facility policies and performing duties in a skilled nursing setting

Nice To Haves

• Prior LTC exposure preferred

Responsibilities

• Collect data relevant to residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team
• Contribute to a developmentally appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions, involving the resident/family and healthcare team
• Recommend revisions to the plan of care after evaluating resident outcomes; identify referrals to address all aspects of care
• Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse; collaborate with physicians and ancillary staff; maintain a safe, healthy environment
• Maintain current knowledge and competency in nursing practice; participate in mentoring or orientation of staff; engage in continuing education and inservice programs
• Demonstrate ability to care for geriatric residents and adapt care to adult and geriatric patient needs
