About The Position

This is a travel assignment for a Licensed Practical Nurse (LPN) in Long Term Care, located in Rochester, NY. The assignment begins on 09/14/2026 and lasts for 17 weeks. Requirements

Active New York LPN license
BLS certification (American Heart Association)
A minimum of 2 years’ experience in long-term care
Experience with wound care; ability to manage geriatric residents in a long-term care setting
Strong critical thinking skills and ability to work independently on a rotating 12-hour schedule

Responsibilities

Provide direct patient care to geriatric long-term care residents, including medication administration and oversight of CNA colleagues
Monitor residents’ conditions, document changes, and communicate with the supervising nurse as needed
Perform wound care and dressing changes, with attention to healing and infection prevention
Assess patient needs, contribute to or review care plans, and ensure care is delivered with dignity and respect
Maintain accurate documentation and adhere to unit policies, safety standards, and shift handoffs
