About The Position

This is a travel assignment for a Licensed Practical Nurse (LPN) in Long Term Care at a Skilled Nursing Facility in Geneva, NY. The assignment is for 26 weeks, starting on 09/15/2026, with a weekly pay range of $729-$775. The role involves providing direct and indirect skilled nursing care to residents under the supervision of a Registered Nurse (RN).

Requirements

Active New York LPN license
BLS certification (AHA or RQI accepted)
Minimum 1 year of LPN experience in a long-term care facility
Ability to work the assigned shift (2x8 nights) and comply with every-other-weekend/evening/Holiday requirements
Demonstrated ability to provide geriatric-focused nursing care and collaborate with an interdisciplinary team
Effective communication skills and ability to document and report resident status accurately

Responsibilities

Provide direct and indirect skilled nursing care to residents on the assigned unit under RN supervision.
Collect data on residents’ physical and psychosocial condition and communicate findings to the RN and the Interdisciplinary Team.
Contribute to development of an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve the resident/family and healthcare team.
Recommend revisions to the care plan after evaluating resident outcomes and coordinate referrals as needed to address all aspects of care.
Collaborate with residents/families and Interdisciplinary Team to ensure safety, quality of care, and customer satisfaction.
Maintain current nursing knowledge and competence; participate in mentoring/orientation of staff; demonstrate leadership under the direction of the RN.
