About The Position

This is a travel assignment for a Licensed Practical Nurse (LPN) in Long Term Care, located in Gallup, New Mexico. The assignment is temporary and will be at a Skilled Nursing Facility. The contract duration is 7 weeks, with a start date of 09/01/2026. The role involves providing direct resident care, administering medications, monitoring patient responses, and collaborating with the healthcare team.

Requirements

Active Licensed Practical Nurse license in New Mexico (or eligibility for NM licensure).
Experience in long-term care or a skilled nursing facility preferred.
Current CPR/BLS certification preferred.
Ability to work 12-hour shifts, with a 3x12-day schedule and 36 guaranteed hours per week.
Available to start 09/01/2026 for a 7-week assignment.
Travel - temporary assignment eligibility; local assignment status noted in the posting.

Responsibilities

Administer prescribed medications and monitor patient responses.
Provide direct resident care, including hygiene, mobility assistance, and activities of daily living.
Monitor vital signs, assess for changes, and report findings to the supervising RN promptly.
Document care activities accurately and maintain up-to-date resident records.
Collaborate with the interdisciplinary team to implement and adjust care plans as assigned.
