About The Position

CLFC Healthcare and Communications is hiring a W-2 per diem Licensed Practical Nurse for an on-site county-operated, 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has an RN on every shift. LPNs administer medications and treatments on their unit, observe residents and report changes to the charge RN, and support CNAs within New York scope of practice. The position offers $38.00–$42.00 per hour, paid weekly. Experience determines the rate within the range; night and weekend shifts pay at the top of the range. Overtime over 40 hours and all ten County holidays are paid at time and a half. Per diem nurses choose available shifts through the BRIDGE App, with no minimum hours. Shifts include eight paid hours with a half-hour unpaid meal break, and twelve-hour shifts may be available. Orientation is paid at the nurse’s rate. A confirmed shift canceled by the facility two hours or less before it begins is paid for four hours. The role includes reimbursement for New York license and CPR renewals, support for continuing education and certifications, and up to $1,500 per year in tuition assistance toward an RN. A $500 referral bonus is offered for bringing a nurse or aide onto the roster. Fingerprinting, immunizations, screening, and license verification are arranged and paid by CLFC. Paid sick leave accrues under New York law, and professional liability insurance covers each shift. Program terms are provided in writing before acceptance.
