About The Position

CLFC Healthcare and Communications is hiring per diem Licensed Practical Nurses for an on-site, 192-bed county skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has an RN on every shift. LPNs administer medications and treatments on their unit, with on-site leadership and an RN available on every shift. This W-2 hourly position offers a self-selected schedule with no minimum hours; nurses may choose two shifts a month or twenty. Available eight-hour shifts are 6:30 AM–3:00 PM, 2:30 PM–11:00 PM, and 10:30 PM–7:00 AM, with twelve-hour shifts when available. Shifts may be available any day of the week. Each eight-hour shift includes a half-hour unpaid meal break. Open shifts are posted in the BRIDGE App, where nurses select shifts and receive confirmation. Compensation is $38–$42 per hour, with experience determining the rate and nights and weekends paid at the top of the range. Overtime over 40 hours and all ten County holidays are paid at time and a half. Pay is weekly, and orientation is paid at the nurse’s rate. If the facility cancels a confirmed shift two hours or less before its start, the nurse is paid for four hours; with more notice, the shift is removed from the schedule. The position includes reimbursement for New York license renewal and CPR renewal, support for continuing education and certifications, and up to $1,500 per year in tuition assistance toward an RN. A $500 referral bonus is offered for referring a nurse or aide who joins the roster. Fingerprinting, immunizations, screening, and license verification are arranged and paid for by CLFC. Paid sick leave accrues under New York law, and CLFC provides professional liability insurance for every shift. Program terms are provided in writing before acceptance. A Program Manager is available to help with scheduling and pay questions. CLFC also considers rostered nurses first for new assignments when they arise.
