About The Position

This is a travel assignment for a Licensed Practical Nurse specializing in Long Term Care within a Skilled Nursing Facility. The role involves administering medications, monitoring residents' status, assisting with daily living activities, performing standard nursing procedures, maintaining clinical documentation, and ensuring safety and compliance with regulatory standards. The assignment is for 15 weeks, with a shift pattern of 3x12-hour shifts per week, totaling 36 guaranteed hours.

Requirements

Active Licensed Practical Nurse license (or eligible for travel assignment)
Experience in a long-term care or skilled nursing setting preferred
Ability to work the listed shift pattern (3x12-hour shifts) and maintain reliability
Strong communication and teamwork skills
Proficiency with basic computer tasks and resident documentation
Willingness to comply with onboarding requirements and facility policies

Responsibilities

Administer prescribed medications and document administration in accordance with policy
Monitor residents’ status, take routine vital signs, and report changes to the supervising RN
Assist with activities of daily living and provide comfort measures to residents

Maintain accurate clinical documentation in the electronic health record and contribute to care planning
Ensure safety, infection control, and compliance with regulatory standards
