About The Position

This is a travel Licensed Practical Nurse position in Long Term Care, located in Waterloo, NY. The role is for a skilled nursing facility and requires coverage during the holiday season. Full-shift orientation begins on a Tuesday. The contract duration is 23 weeks, with a start date of 09/15/2026. The position is full-time, with a shift of 5x8s.

Requirements

NY LPN licensure (active)
BLS certification (AHA or RQI)
Minimum 1 year experience as an LPN
Experience serving in an LPN capacity at a long-term care facility
Ability to provide care to adult and geriatric residents
Must be able to participate in holiday season coverage and start full-shift orientation on a Tuesday

Responsibilities

Provide direct and indirect skilled nursing care for residents, under the supervision of a Registered Nurse.
Collect data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team.
Contribute to the development of an age-appropriate, culturally relevant plan of care, involving the resident, family, and healthcare team to achieve outcomes.
Recommend revisions to the plan of care based on resident outcomes and coordinate referrals as needed.
Collaborate with residents/families and interdisciplinary team to ensure quality care and a safe environment.
Maintain current knowledge and competency in nursing practice; participate in mentoring or orientation of staff; demonstrate leadership under supervision.
