About The Position

EmpowerMe Wellness is hiring a part-time Licensed Physical Therapist Assistant to provide therapy and wellness services to older adults through EmpowerMe 360, an integrated model connecting therapy, wellness, and medical providers. The role helps senior residents live stronger, safer, and more independent lives through clinical care, care coordination, and wellness support.

The therapist works collaboratively with residents, caregivers, and other providers, helping residents improve independence, confidence, and satisfaction. The position offers predictable schedules without late nights or constant driving, along with opportunities for leadership, specialty programs, and professional development.

Requirements

• Current professional license in the applicable therapy discipline, including PTA.
• Passion for empowering seniors to live their best lives.
• Strong clinical skills and a collaborative spirit.

Responsibilities

• Provide therapy and wellness services for older adults.
• Conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team.
• Support care coordination through wellness check-ins, caregiver communication, and telehealth support.
• Maintain professional and ethical standards in line with company initiatives.
• Contribute to program development and continuous quality improvement.
