Licensed Physical Therapist Assistant (PTA) – Skilled Nursing Facility (SNF) – San Jose, CA
About The Position
Relient Health is seeking a caring and compassionate Physical Therapist Assistant to work full-time at a Skilled Nursing Facility in San Jose, California. The facility provides rehabilitation services through a collaborative, patient-focused therapy department. The position offers a supportive team environment, modern equipment, mentorship, and opportunities for professional growth. New graduates and experienced PTAs are welcome. This is a direct-hire, permanent-placement role scheduled Monday through Friday. The estimated rate is approximately $44–$48 per hour, depending on experience, setting, and location. Requirements
- Graduate of an accredited Physical Therapist Assistant program.
- Current California license or eligibility to obtain one.
- Strong communication, documentation, and interpersonal skills.

Responsibilities
- Provide therapy services under the direction of a licensed Physical Therapist.
- Assist in developing and implementing individualized treatment plans.
- Document patient progress using EMR systems.
- Communicate with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and nursing staff.
- Promote patient independence and mobility.
