About The Position
EmpowerMe Wellness is hiring a Licensed Occupational Therapist to provide therapy and wellness services to older adults at Aegis Living - Aptos in Aptos, California. The role is part of EmpowerMe 360, an integrated model connecting therapy, wellness, and medical providers to support residents in living stronger, safer, and more independent lives. The therapist conducts assessments, develops personalized care plans, and delivers evidence-based interventions as part of a collaborative team. The position also supports care coordination through wellness check-ins, caregiver communication, and telehealth support, while contributing to program development and continuous quality improvement.
