Licensed Occupational Therapist (OT)
About The Position
EmpowerMe Wellness is seeking a Licensed Occupational Therapist to provide therapy and wellness services for older adults through EmpowerMe 360, an integrated model connecting therapy, wellness, and medical providers. The role supports residents in living stronger, safer, and more independent lives through assessments, personalized care plans, evidence-based interventions, care coordination, and wellness check-ins. The therapist works collaboratively with caregivers and other providers, contributes to program development and quality improvement, and helps residents maximize independence, confidence, and satisfaction. The position offers predictable schedules without late nights or constant driving, along with opportunities for leadership, specialty programs, and professional development. It combines therapy, care coordination, and wellness check-ins in a supportive, team-focused environment. The stated pay range is $50.00–$61.00 per hour.
Requirements
- Hold a current professional license in the relevant therapy discipline; occupational therapy is among the listed disciplines.

Responsibilities
- Provide therapy and wellness services for older adults.
- Conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team.
- Support care coordination through wellness check-ins, caregiver communication, and telehealth support.
- Maintain professional and ethical standards in line with company initiatives.
- Contribute to program development and continuous quality improvement.
