Licensed Occupational Therapist (OT) PRN or Part-Time
About The Position
EmpowerMe Wellness is hiring a Licensed Occupational Therapist to provide therapy and wellness services to older adults as part of EmpowerMe 360, an integrated model connecting therapy, wellness, and medical providers. The role helps residents live stronger, safer, and more independently through personalized, evidence-based care and coordinated support. The therapist conducts assessments, develops individualized care plans, delivers interventions, and collaborates with the care team. The position also includes wellness check-ins, caregiver communication, telehealth support, contributions to program development and quality improvement, and work to help residents maximize independence, confidence, and satisfaction. Requirements
