About The Position

Urban Health Plan is seeking a Licensed Mental Health Counselor to join its Behavioral Health Department. Clinical associates provide psychotherapy to patients, including individual therapy. The counselor will work with adults and families and promote patients’ health and wellness through evidence-based clinical therapy. The role involves working in a fast-paced, interdisciplinary environment and collaborating with medical and mental health providers.

Requirements

• Master’s degree in Mental Health Counseling from an accredited institution.
• New York State LMHC credential.
• Two years of psychotherapy experience.
• Bilingual English/Spanish proficiency.
• Ability to thrive in a fast-paced environment as a member of an interdisciplinary team.
• Ability to identify high-risk psychosocial factors in patients and families that impact health status.

Responsibilities

• Complete behavioral health screenings and assessments and provide evidence-based psychotherapy.
• Provide point-of-care services when necessary.
• Conduct therapy sessions with patients.
• Support treatment plans and collaborate with other mental health providers to provide care.
• Engage in crisis intervention.
• Collaborate with medical providers and provide consultations.
• Identify high-risk psychosocial factors in patients and families that affect health status.
