About The Position

Enhabit Home Health & Hospice is hiring a Licensed Master Social Work (LMSW) to provide hospice social services on a PRN basis. The role administers social services to patients who need intermittent professional services and teaches patients, families, and other members of the health care team. Services must be performed in accordance with physician orders and the established plan of care. Requirements

Master’s degree from a school of social work accredited by the Council on Social Work Education.
Must be certified or licensed in the state where currently practicing, subject to the state-specific qualifications listed in the posting.
At least one year of social work experience in a health care setting.
Basic demonstrated technology skills, including operation of a mobile device.
Valid state driver license.
Maintain automobile liability insurance as required by law.
Maintain dependable transportation in good working condition.
Ability to safely drive an automobile in all types of weather conditions.
CPR certification.

Nice To Haves

Medicare home health or hospice experience is preferred.
