About The Position
Enhabit Home Health & Hospice is hiring a Licensed Master Social Work (LMSW) professional to provide social services to patients receiving intermittent professional services. The role includes teaching patients, families, and other members of the health care team. Services must be provided in accordance with physician orders and the established plan of care. The posted compensation is $75 per evaluation, $60 per routine visit, and an hourly rate of $60. The position is PRN. Requirements
- Master’s degree from a school of social work accredited by the Council on Social Work Education.
- Must be certified or licensed in the state where currently practicing, subject to the state-specific requirements listed in the posting.
- At least one year of social work experience in a health care setting.
- Basic demonstrated technology skills, including operation of a mobile device.
- Valid state driver license.
- Maintain automobile liability insurance as required by law.
- Maintain dependable transportation in good working condition.
- Ability to safely drive an automobile in all types of weather conditions.
- CPR certification.
Nice To Haves
- Medicare home health or hospice experience is preferred.
