Licensed Master Social Work (LMSW) - Hospice - PRN
About The Position
Enhabit Home Health & Hospice is hiring a Licensed Master Social Work professional to provide hospice social services on a PRN basis. The role administers social services to patients requiring intermittent professional services and provides education to patients, families, and other members of the health care team. Services must be provided in accordance with physician orders and the established plan of care. Applicants must have a master’s degree from a Council on Social Work Education-accredited school, applicable state certification or licensure, at least one year of social work experience in a health care setting, and basic technology skills, including use of a mobile device. State-specific credential requirements vary. Medicare home health or hospice experience is preferred. The position requires a valid state driver license, legally required automobile liability insurance, dependable transportation in good working condition, the ability to drive safely in all weather conditions, and CPR certification.
Requirements
- Master’s degree from a school of social work accredited by the Council on Social Work Education.
- Certification or licensure in the state where the employee practices, subject to the state-specific requirements listed in the posting.
- At least one year of social work experience in a health care setting.
- Basic demonstrated technology skills, including operation of a mobile device.
- Valid state driver license.
- Maintain automobile liability insurance as required by law.
- Maintain dependable transportation in good working condition.
- Ability to safely drive an automobile in all types of weather conditions.
- CPR certification.
