Lead Patient Coordinator
About The Position
At Great Strides Rehabilitation, an H2 Health company, the Lead Patient Coordinator is the first point of contact for patients and families at outpatient clinics and helps create a positive, efficient, and supportive experience. The lead role is responsible for administrative compliance and Patient Coordinator support in the Jacksonville area, including training and directing other Patient Coordinators in person and virtually. The position operates at the St. Augustine Great Strides Rehabilitation location and requires occasional travel to other Great Strides locations. The Lead Patient Coordinator works closely with the Regional Administrative Manager to support personal, professional, and company goals. The role helps ensure smooth day-to-day clinic operations through scheduling, patient check-in and check-out, insurance verification, and communication with clinical teams.
Requirements
- High school diploma or equivalent.
- Excellent organizational and communication skills.
- Friendly, reliable, and detail-oriented.
- Comfortable using scheduling and EMR systems.
- Ability to manage multiple tasks and priorities.
Nice To Haves
- Previous experience in a healthcare or customer-facing environment is preferred.
Responsibilities
- Greet patients and families warmly and professionally.
- Manage patient check-in and check-out procedures.
- Schedule appointments and maintain accurate records.
- Verify insurance benefits and collect co-pays as needed.
- Support communication between therapists and patients.
- Perform general administrative tasks to keep the clinic running smoothly.
- Support administrative compliance and Patient Coordinator operations in the Jacksonville area.
- Train and direct other Patient Coordinators in person and virtually.
- Work closely with the Regional Administrative Manager to help meet personal, professional, and company goals.
- Work at the St. Augustine Great Strides Rehabilitation location and occasionally travel to other Great Strides locations.
