The Kidney Care Specialist (KCS) supports patients with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) through early education, care navigation, and ongoing engagement. The role bridges the gap between diagnosis and informed treatment choice by providing timely, consistent education and support across the kidney care continuum. Responsibilities combine field-based physician and community outreach with virtual patient support; deployment may be field-based, virtual, or hybrid depending on market needs. The role requires local or regional travel to meet patients and nephrology providers, and candidates in field-based roles must live within or near the assigned territory. The KCS educates patients about kidney disease progression, treatment options—including home modalities and transplantation—and self-management strategies. The specialist assesses patient needs, readiness, barriers, and preferences; collaborates with nephrologists and care teams; supports referrals and follow-through; and connects patients with appropriate resources. The role also documents patient interactions, tracks engagement and program performance, contributes to continuous improvement of education materials and workflows, and adheres to regulatory, privacy, safety, and documentation standards. There are no direct supervisory responsibilities. The position works under the guidance of Kidney Care leadership and in close collaboration with clinical and operational teams.
