The Kidney Care Specialist (KCS) supports patients with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) through early education, care navigation, and ongoing engagement. The role bridges the gap between diagnosis and informed treatment choice, helping patients receive timely, consistent education and support across the kidney care continuum. Responsibilities combine field-based physician and community outreach with virtual patient support; deployment may be field-based, virtual, or hybrid depending on market needs. The specialist educates patients about kidney disease progression, treatment options—including home modalities and transplantation—and self-management strategies. The role assesses patient needs, readiness, barriers, and preferences; coordinates with nephrologists, care teams, and internal partners; supports referrals and follow-through; and connects patients with appropriate resources. The specialist documents interactions and outcomes, tracks engagement and program performance using approved systems and tools, and supports compliant, continuous improvement of education materials and workflows. This position requires local or regional travel to meet patients and healthcare providers, and travel to regional business units, corporate meetings, and on-site training. Occasional evening and weekend work may be needed. The position has no direct supervisory responsibilities and works under the guidance of Kidney Care leadership in collaboration with clinical and operational teams.
