The Kidney Care Specialist (KCS) supports people with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) through early education, care navigation, and ongoing engagement. The role combines outreach to physicians and communities with virtual patient support, helping patients move from diagnosis toward informed treatment decisions and receive timely, consistent education across the kidney care continuum. Responsibilities may be field-based, virtual, or hybrid depending on market needs. The specialist provides education on kidney disease progression, treatment options—including home modalities and transplantation—and self-management. The role collaborates with nephrology providers, care teams, and internal partners; supports referrals and coordinated workflows; documents interactions and outcomes; tracks engagement and program performance; and connects patients with appropriate resources. The position has no direct supervisory responsibilities and works under the guidance of Kidney Care leadership.
