Kidney Care Specialist

Fresenius Medical Care•College Park, GA

About The Position

The Kidney Care Specialist (KCS) supports patients with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) through early education, care navigation, and ongoing engagement. The role bridges the gap between diagnosis and informed treatment choice by providing timely, consistent education and support across the kidney care continuum. It combines field-based physician and community outreach with virtual patient support; deployment may be field-based, virtual, or hybrid depending on market needs. The KCS works closely with nephrology providers, advanced practice providers, care teams, internal partners, and referring practices to improve patient outcomes, address care gaps, coordinate referrals, and connect patients with appropriate resources. The specialist provides education on disease progression, treatment options—including home modalities and transplantation—and self-management strategies; supports patients longitudinally; and tracks engagement and program outcomes using approved tools and systems. The role follows applicable regulatory, privacy, safety, and documentation standards and has no direct supervisory responsibilities.
