Kidney Care Specialist

Fresenius Medical Care•Virginia Beach, VA

About The Position

The Kidney Care Specialist (KCS) supports patients with Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) through early education, care navigation, and ongoing engagement. The role bridges the gap between diagnosis and informed treatment choice, providing education and support across the kidney care continuum through field-based physician and community outreach and virtual patient support. Depending on market needs, the role may be field-based, virtual, or hybrid. Field-based work requires local or regional travel to meet patients and nephrology providers and may include extensive travel to physician groups, healthcare locations, regional business units, corporate meetings, and on-site training. The position requires occasional evening and weekend work as needed. The specialist works under the guidance of Kidney Care leadership and collaborates with clinical and operational teams; there are no direct supervisory responsibilities. The role provides patient education about kidney disease progression, treatment options—including home modalities and transplantation—and self-management. It assesses patient needs, readiness, barriers, and preferences; supports coordinated care and referrals; maintains relationships with referring practices; documents interactions and outcomes; and uses virtual tools and reports to monitor engagement and program performance. The specialist also helps improve educational materials and workflows, connects patients with resources, supports quality and compliance standards, and provides longitudinal follow-up in collaboration with the care team. A high school diploma or GED is required. The posting lists a minimum of 1–3 years of experience in kidney care, chronic disease management, or related healthcare fields. A clinical background of 3–5 years, relevant licensure or certification, experience with CKD, ESRD, dialysis, or chronic disease populations, and experience delivering virtual or telephonic patient education are preferred. A valid driver’s license, reliable transportation, and ability to travel within the assigned market are required. The listed hourly pay ranges are $20–$27 and $20–$30; pay depends on work location and qualifications. Benefits include medical, dental, and vision insurance, a 401(k) with company match, paid time off, and parental leave.
