About The Position

The Intake Coordinator coordinates patient referrals and serves as a resource for referral sources. The role collaborates with branch and sales teams to support appropriate coordination and transition to services, and applies knowledge of regulatory guidelines and governing processes.

Requirements

• High school diploma or equivalent.
• At least two years of experience in the health care industry.
• At least one year of experience in home health or pediatrics.
• Demonstrated customer service experience.
• Demonstrated knowledge of Medicare guidelines governing home health or hospice.
• Basic demonstrated technology skills, including operating a mobile device.
• Valid state driver license.
• Maintain automobile liability insurance as required by law.
• Maintain dependable transportation in good working condition.
• Ability to safely drive an automobile in all types of weather conditions. For employees located in Oregon, driving requirements do not apply unless the employee has a clinical license.

Nice To Haves

• Graduate of an approved school of professional nursing and licensed in the state where currently practicing; may be a registered nurse or licensed professional/vocational nurse.
• Experience with intake responsibilities.
• Experience working with a multi-location company.
