Inpatient Case Manager - Developmental Disabilities Unit
About The Position
Fuller Hospital is a 109-bed licensed private psychiatric facility in South Attleboro, Massachusetts, providing inpatient and outpatient behavioral health services to residents of Massachusetts and Rhode Island. Its inpatient units serve adults with general psychiatric or intellectual disabilities and adolescents with general psychiatric issues. Most units can treat patients with co-occurring psychiatric and substance abuse disorders, and the hospital also offers a Partial Hospitalization Program. The Social Worker is an important member of the inpatient multidisciplinary treatment team. This is a full-time, 40-hour-per-week position on the Developmental Disabilities Unit, providing social work services in an inpatient and partial hospital setting.
Requirements
- Master’s degree in social work, counseling, or another human services-related field.
- Required LCSW, LICSW, LMFT, or LHMC license, or eligibility to receive one of these licenses in the future.
Nice To Haves
- Prior hospital social worker experience is preferred.
- Experience and/or passion for working with a developmental disability patient population is strongly preferred.
Responsibilities
- Provide a comprehensive range of social work services to a variety of patients in an inpatient or partial hospital setting.
- Act as the primary contact with external providers and actively participate in each assigned patient’s treatment plan.
- Take primary responsibility for patient discharge and aftercare planning.
