About The Position

Sentara Norfolk General Hospital is hiring a Hospitalist Inpatient Case Manager RN to work full-time days with rotating weekends. This position works closely with SMG Physicians for the management of complex care within Hospitalists patients, guardianship, assistance with FMLA paperwork completion, EDD utilization and accuracy, readmission reduction, goals of care and coordination of care needs for prolonged stays, and LTSS signature completion compliance. The role partners with Inpatient Case Management to complete arrangements for complex-care transitions. The Registered Nurse is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. This role directly supports inpatient patients identified in need of comprehensive case management. It promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. The role develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only.

Requirements

  • 12 months nursing experience
  • Registered Nurse
  • Basic Life Support (within 90 days)

Nice To Haves

  • BSN-preferred
  • Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility
  • 1 year acute care case management preferred

Responsibilities
