About The Position

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager RN to work full-time 40 hours per week. The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay (LOS). This role directly supports in-hospital patients identified as needing comprehensive case management. The Inpatient Case Manager RN promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. They also develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver. This position functions exclusively in hospital practice settings.

Requirements

  • 12 months nursing experience
  • Registered Nurse
  • Basic Life Support (within 90 days)
  • Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility

Nice To Haves

  • BSN-preferred
  • 1 year acute care case management preferred
