About The Position

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN)! This position is eligible for up to a $20,000 sign-on bonus for qualified candidates. This is a Full-Time, permanent position (40 hours) with standard working hours of 8:00 AM – 4:30 PM EST, Monday–Friday, with rotating weekends (Saturday or Sunday). A weekday off is provided when working a weekend day so hours do not exceed 40 per week. This is an onsite position at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507. The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. This role will directly support those hospital patients identified in need of comprehensive case management. The Case Manager promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. They develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. This function is performed in a hospital setting only.

Requirements

  • Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
  • BLS within 90 days of hire.
  • Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
  • 1 year of nursing experience

Nice To Haves

  • Bachelor's Degree in Nursing preferred
  • 1 year acute care case management preferred
