About The Position

Sentara Virginia Beach General Hospital is hiring an Inpatient Case Manager RN to work full-time. This role is responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay. The Inpatient Case Manager RN will directly support hospital patients identified as needing comprehensive case management. This position promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. The role also involves developing and/or implementing a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver. This position functions exclusively in hospital practice settings.

Requirements

  • 12 months nursing experience
  • Registered Nurse
  • Basic Life Support (within 90 days)
  • Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility

Nice To Haves

  • BSN-preferred
  • 1 year acute care case management preferred

Responsibilities

  • Directly support those in hospital patients identified in need of comprehensive case management.

  • Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.
  • Functions in one of the following practice settings: Hospitals Only
