Sentara Virginia Beach General Hospital is hiring an Inpatient Case Manager RN to work full-time. This role is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay. The Inpatient Case Manager RN will directly support hospital patients identified in need of comprehensive case management, promote effective utilization and monitoring of health services, and collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions. They will develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver, functioning exclusively in hospital settings.
