This full-time role is located at Ascension All Saints Spring Street Campus in Racine, WI, within the Case Management Department/Specialty, on a day schedule.

Requirements

  • Licensed Registered Nurse credentialed from the Wisconsin Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Responsibilities

  • Lead the creation of personalized care plans by integrating patient values and clinical needs to ensure a seamless recovery journey from admission to discharge.
  • Advocate for your patients by identifying and removing barriers to care, ensuring they have the resources and support needed for a safe transition home.
  • Drive clinical excellence by evaluating medical necessity and documentation, ensuring every patient receives the right level of care at the right time.
  • Collaborate with multidisciplinary teams and payers to optimize benefits and coordinate post-acute services, acting as the vital link between the hospital and the next phase of healing.
