How you’ll make an impact in this role Care Orchestration: Design and manage comprehensive care plans across the continuum, proactively removing barriers to recovery and advocating for necessary resources. Clinical Oversight: Monitor medical necessity and appropriate levels of care through continuous assessment and medical record review. Strategic Collaboration: Maintain a constant feedback loop with supervisors and transition teams to refine care plans and ensure seamless handoffs. Resource Advocacy: Serve as a subject matter expert on documentation and post-discharge options, acting as a vital link between patients and post-acute service providers. Utilization Management: Partner with payers to optimize benefits and ensure the efficient, effective use of clinical resources.

Requirements

  • Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
