Sentara Princess Anne Hospital is hiring a full time RN Case Manager for its inpatient units house-wide. A generous Sign on Bonus is available for qualified candidates. This position is ideal for individuals who enjoy providing comprehensive care to patients and families throughout their inpatient stay, developing strong relationships, and acting as a clinical leader within the healthcare team. The role involves driving decisions to ensure the safest and most effective transition plans for patients and families, collaborating with a team of nurses and social workers, and contributing to hospital and organizational strategic initiatives to continuously improve patient care. The Case Manager RN will utilize complex critical thinking skills and is responsible for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay. The role directly supports hospitalized patients identified as needing comprehensive case management, promotes effective utilization and monitoring of health services, and collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. The Case Manager develops and/or implements a comprehensive care plan based on assessment and evaluation, addressing the physical, mental, and psychosocial needs of the inpatient/caregiver. This role functions exclusively in hospital practice settings.
