About The Position

Sentara Princess Anne Hospital is hiring a full time RN Case Manager for its inpatient units house-wide. A generous Sign on Bonus is available for qualified candidates. This position is ideal for individuals who enjoy providing comprehensive care to patients and families throughout their inpatient stay, developing strong relationships, and acting as a clinical leader within the healthcare team. The role involves driving decisions to ensure the safest and most effective transition plans for patients and families, collaborating with a team of nurses and social workers, and contributing to hospital and organizational strategic initiatives to continuously improve patient care. The Case Manager RN will utilize complex critical thinking skills and is responsible for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay. The role directly supports hospitalized patients identified as needing comprehensive case management, promotes effective utilization and monitoring of health services, and collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. The Case Manager develops and/or implements a comprehensive care plan based on assessment and evaluation, addressing the physical, mental, and psychosocial needs of the inpatient/caregiver. This role functions exclusively in hospital practice settings. Requirements

  • A minimum of 12 months of nursing experience is required

Responsibilities

  • Providing care to the patient and family over the entire inpatient stay
  • Developing a relationship with patients and families
  • Acting as a clinical leader of the healthcare team
  • Driving decisions to assure the safest, most effective transition plan for patients and families
  • Working in a team of nurses and social workers, using each other's expertise
  • Helping to drive hospital and organizational strategic initiatives in always improving patient care
  • Utilizing complex critical thinking skills
  • Responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS
  • Directly support those in hospital patients identified in need of comprehensive case management
  • Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions
  • Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver
  • Functions in one of the following practice settings: Hospitals Only
